Scalpels Blades
- The No.20 is a large version of the No.10 blade with a curved cutting edge and a flat, unsharpened back edge.
- Used for orthopaedic and general surgical procedures it fits handle No. 4, 4L, 4 Graduated & 6B.
With three models to choose from, namely the Braithwaite, Cobbett and Watson, these Stainless Steel Handles are used in conjunction with the sterile 158mm Skin Graft Blades for harvesting "Macro" or "Major" Skin Grafts. The handles have an in-built adjustable guard which can limit the maximal thickness of the graft harvested depending on the properties of the skin at the donor site. This can vary in thickness due to multiple factors including the age of the patient.
The exact thickness of the graft can also depend upon:
1) The contact angle between the blade and skin whilst harvesting;
2) The pressure applied by the operator onto the handle whilst harvesting.
These are important aspects of skin grafting technique that need to be mastered to enable reproducibly consistent harvesting of high quality split thickness skin grafts.
The Braithwaite Handle (1955 Fenton Braithwaite) was a modification of the Humby and Bodenham Knives and was the first that did not need to be dismantled in order to replace the blade. The adjustment between the roller and the blade itself is achieved by two knurled collars mounted on the back of the handle. The Stainless steel blade is locked securely in the leaf spring holder. To fit a new blade one moves the safety catch in order to open the leaf and the blade can be inserted under the roller before reengaging the safety catch. There is a tendency for skin to roll up around the guard when cutting thin split skin grafts with the Braithwaite Handle however this can be corrected by the placing traction on the cut skin otherwise the fine sheet of skin may tear. The roller must also move freely from side to side requiring slack end-bearings that can reduce precision in the setting resulting in the graft sometimes becoming gradually thicker as the handle progresses. Two adjustments are necessary at either end of the handle to set the depth of cut and again if the setting is unequal the graft may be thicker on one side.

Supporting existing safe passing protocols and procedures the scalpel has a reinforced temporary lock in the forward user position ensuring the blade will stay extended even if it encounters a hard surface or bone during use. When in the retracted position the slider mechanism contains a notch that holds the blade securely in the retracted position. A permanent lock position at the rear of the scalpel, requiring a conscious amount of force to engage the slider, ensures the contaminated blade remains retracted whilst disposing in an appropriate sharps container at the end of the procedure.

The Silvers stainless steel "miniature" skin graft handle in a distinctive blue was originally developed by H.L. Silver in Toronto, Canada.
Combined with our surgical stainless double edged razor blades it is ideal for harvesting small intricate grafts from areas which would not be accessible with the larger standard knives such as the ear, dorsa of the hands or scalp.
Although used mainly within Plastic, Reconstructive and Oral surgery the Silver's can also be used for the debridement of wounds.
Instructions for Use *:
1) Under aseptic / sterile operating conditions the blade is inserted into the handle being careful not to damage the cutting edge of the blade by striking it to metal. Also be careful not to injure yourself as the blade is very sharp.
2) The blade retaining plate is then fitted. Note that it is asymmetrical, the shorter section is applied over the free blade edge. Incorrect fitting of the blade retaining plate so that the retaining plate overlies the free edge of the blade means it can strike the handle body near the lateral free edges of the blade.
3) Insert and tighten the fixing nut to firmly hold the blade in the handle.
4) The thickness of the graft to be harvested is determined by adjusting the two thumb screws on each upper side of the handle.The thumb screws move the harvesting guide either closer to, or further from the blade and change the thickness of the harvested graft accordingly. Turn thumb screws equally to maintain the blade parallel to the harvesting guide.
5) Split thickness grafts vary in thickness from thin (0.008 of an inch) to medium (0.010 of an inch) to thick (0.012 of an inch).
6) A Swann-Morton No.15 scalpel blade can be passed between the blade and guard to check selected thickness prior to harvesting a graft. The thickness of the disposable 15 blade is 0.394mm or 0.0155 inch. If the number 15 blade passes freely between the skin graft disposable blade and the harvesting guide this will provide a thick split thickness skin graft. This is useful if harvesting from the anterior or lateral upper thigh.
7) If the forearm or arm is chosen select a thin to medium thickness split thickness skin graft. The number 15 disposable blade will only pass part way to the bevel. The recipient site will determine how thick a graft you wish to harvest.
8) After appropriate anaesthesia and application of lubrication such as liquid paraffin to the donor site and skin graft knife, the graft is harvested by steadily moving the blade back and forth at the same time stretching the skin to hold it firm.

The Cobbett Handle (1968 J. Cobbett) offered a solution to the drag whilst still preventing the skin rolling around the guard. The guard slides to and fro but does not rotate. The Braithwaite Handle was modified by cutting a "D" section instead of a circular hole in the a guard end of one of the cam levers and grinding a corresponding flat on the side of the end of the guard. The modified handle has little drag and no tendency for the skin to roll up on the guard.

The Watson Handle (1960 John Watson) addresses some of these issues as it has a fixed, polished, stainless steel guard and a simpler, more rigid thickness adjustment. The rod has no independent movement, either sliding or rotation, while the graft is being taken. At its end-bearings, it passes eccentrically through the bushes, hence the depth of cut can be set by rotation of the rod with the single knurled control knob, the rod remaining parallel to the blade settings. The bearings therefore have a precision fit and there is no "play" in use. While taking the graft, spontaneous rotation of the rod is slight, but the locking lever is provided to avoid accidental alteration of the setting once it has been made. The fixed guard reduces skin rolling however introduced a considerable degree of "drag" since it cannot slide to and fro with the skin surface. As a result the edges of both the graft and donor site can be ragged an effect that can only partially be avoided by lubricating with liquid paraffin.

The "new" custom designed surgical stainless steel Silver's blade has a 11/2 inch (4 cm) postage stamp size cutting edge to be used with the Silvers miniature Skin Graft Knife handles.
